Application ProcessParents who wish to enroll their children must fill out an application at CRT:• Head Start Center Locations• Family Service Centers • 555 Windsor St., Hartford Enrollment OfficeParents must bring the following documentation for enrollment:• Income Verification for household for one month (for example: Tax Forms, W-2 Form, Unemployment compensation, Pay stubs) • Medical insurance card • Proof of address • Updated physical exam (within one year) • Birth Certificate• Social Security Card
