How to Apply
Applications accepted Monday â€“ Friday, 8 a.m. â€“ 5 p.m., at CAP Tulsaâ€™s main office, 4606 S. Garnett Road, in the Exchange Center East Building, 1st Floor. Applications must be completed by a parent or legal guardian.
To complete a childâ€™s application, parents or guardians must bring:
Birth Certificate for each child applying
Current Photo ID of parent/guardian completing the application
Proof of current address
Proof of family income (for both parents, if both parents are employed or if custody is shared)
Proof of current immunizations
Childâ€™s proof of health insurance (if available)
Current Well-Child Checkup (if available)
(Applications that do not include the documents above will not be accepted)
Acceptable Proofs of ID include one of the following documents with a current picture of the parent/guardian completing the application:
Government issued photo ID from the U.S. or any country
Resident ID card
Work permit card
High School ID
