How to Apply
Applications are taken year round. The following information must be provided to your local Head Start Center to complete the application process:
Proof of income
Proof of birth
Child's up-to-date immunization record
Medicaid or health insurance information
Birth dates of all family members
If applicable, a copy of the MDT and IFSP/IEP
