How to Apply
Please include copies of the following documents:
Your child’s Birth Certificate
Proof of Residence
W-2 Pay Stub
We will contact you when we receive your application to make an appointment so we may talk about enrolling your child in Early Head Start. When you come to your appointment, you will need to bring the following documents with you:
Record of your child’s latest physical
Immunization record
TANF Card (if applicable)
WIC Card
Your child’s Social Security Card
Alpi (if applicable)
Proof of Guardianship (if applicable)
M.C. Readiness
Medicare/Florida Kidcare
Food Stamp Card (if applicable)
Parent ID
