How to Apply
Eligibility
Child must be 3 years old by September 10th
Child must NOT turn 5 before September 10th
Students with a diagnosed disability are eligible on their 3rd birthday
Enroll
Your completed application packet must include:
Completed Child Enrollment Application
Income Verification--For each adult in the household that has a legal relationship to the child or to the child's primary care giver.
Immunization Records--Please contact us if you need any assistance in obtaining documentation.
