THE CHILD’S BIRTH CERTIFICATE 2. THE CHILD’S IMMUNIZATION RECORD 3. PROOF OF RESIDENCY – any one of the following: Voter’s Registration, Driver’s License, Utility Bill or Telephone Bill 4. PROOF OF INCOME – any one of the following: Letter from Current Employer, Last Pay Stub, W2 Form, or TANF stub 5. PHYSICAL FORM - Click Here to Download Form Head Start and Early Head Start are federally funded programs that service primarily low-income children and their families. To be eligible for the Programs, families must qualify according to the income guideline set by the federal government.
