Enrollment Process and Application Form________________________________________ To enroll your child in the Early Childhood Education Program, you must:• Complete the application:
2011-12 Early Childhood Enrollment Application. You may also call the Early Childhood Education office at 412-325-4291 or 412-325-4292 to request an application be mailed to you.• Provide the following documents with your appliction:o A copy of the child's immunization records.o A copy of the child's birth certificate.o A copy of the child's current health, physical and dental records.o Two (2) proofs of residency (i.e. driver's license, utility bill, etc.).o Proof of income for all members of the household.
