Home-based Learning available to children (and their families) prenatal through age 5. A highly trained home-based teacher comes to your home for one and one half hours each week offering family support. Along with the home-based teacher's support, you receive "The Portage Guide Birth to Six" curriculum to assist you in planning activities that promote the development of your child. Child / parent socializations are held twice a month for group learning opportunities.
Who is Eligible?

Children who are 3 to 5 years old are eligible for Head Start services. Pregnant women and children from birth to 3 years of age are eligible for Early Head Start services. Children and families who are homeless, in foster care, or receive TANF or SSI are also eligible for services. Eligibility is determined by Head Start program staff and some families may be eligible for services if they are determined to be at or below the federal poverty level. Some grantees enroll a percentage of children from families with incomes above the Poverty Guidelines as well.

How to Apply
Child Application If you are interested in enrolling your child in any of the programs offered, please complete a child application and return it to Family Forum, Inc., 1507 Tower Ave., Suite 312, Superior, WI 54880. After the application is received, someone from the agency will set up a time to meet with you to do income verification to determine whether or not your child is income eligible for the program. A families income needs to be verified or documented as having zero income on the application according to the Poverty Guidelines. There are many ways, besides meeting the poverty guidelines, to determine if a child is eligible for Head Start. This will be determined at the time of verification.
