How To Apply:
If you would like to give your child a “head start†in life, call the centernearest to where you live for additional information.The Head Start Brochure and the <a href="http://www.middeltacommunityservices.org/HS-001_Head_Start_Application-PDF.pdf">Head Start Enrollment application</a> now available online in PDF format. You will need the Adobe Reader to view and print the brochure and application. If you do not have the reader, click here to download the free reader. No child will be accepted without the following documents attached to the application:*Child's age (Birth Certificate/child must be 3 years old)*Child's Immunization record (Shots)*Child's Social Security number*Household IncomeComplete entire form and return with the above documents to your local Head Start Center.Please us blue or black ink only.

The Head Start Program is a pre-school developmental program which provides early education for three to five year old children. There are eight (8) facilities throughout a three (3) county area which includes Phillips, Monroe and Prairie Counties. Activities include a pre-school education curriculum, parent involvement, social services,health and dental screenings, mental health services, developmentally disabled services,family counseling, and volunteer opportunities.
