Birth Certificate 2. Shot Record 3. Income Verification (2011 Tax Return, OWF, SSI, Child Support verification, Unemployment) 4. Child's Social Security Card 5. Child's Insurance Card 6. We have many Parent Trainings scheduled for this school year on a variety of subjects. Please check your calendar for dates and times of these trainings. We recognize and value that as parents you are your child's first and most influential teacher- make sure you read to your child everyday! Together we will make this a great year!
