How To Apply:: How do I enroll my child?You may call 226-5664 or 937-2-ENROLL to be directed to the nearest location. You will be asked to come in or you can make an appointment to complete the application.What do I have to bring to the application appointment?You will need to bring proof of child's birth, proof on your income and the child's shot record. What are the Health Requirements for my child before they start the program?Children are required to have a physical exam by a doctor. Once accepted into the program, you will receive the health exam form to take to the doctor. Children must also have at least one of each of the required immunizations: 1 MMR, 1 HIB and 1 DPT.
