How To Apply:
Enrollment Is Easy ... 1. Contact the Nearest Center. Click here for a map and list of all Centers. Telephone numbers and email addresses for each center will be under “More Detailsâ€. (If you don’t know which Center to call, dial toll-free 1-800-762-8432.) 2. Meet with the Family Worker to apply. (We do the paperwork!) You’ll need your child’s birth certificate, insurance information, immunization records, and verification of income (such as a recent tax return or paycheck stub). 3.
