Enrollment Forms Below are some of the forms that we use in the enrollment process. After you have called the Administrative Office (801-375-7981 ext. 136) and been put on an interest list, an intake specialist (Family Advocate) will contact you to set up an appointment to apply for the program. At that time they will indicate which of the following forms can be completed prior to your appointment. Please wait until that time to print or complete any of the forms listed below.

Mountainland Head Start provides quality educational services to all enrolled children through part-day center-based classes.We operate the traditional model for Head Start which is the part-day classroom. In this model, between sixteen and seventeen children share a classroom at the center four days per week, for 3 1/2 hours per day.
