How to Apply
The following documents are required:
Proof of Income (the 1040 income tax form is preferred)
Child’s Birth Certificate
Parent and Child Social Security Card
Drivers License
Proof of Auto Insurance
Child’s Medicaid Card
3 Proofs of Residency-MUST BE ORIGINAL DOCUMENTS; COPIES WILL NOT BE ACCEPTED
Immunization (shot) Record
Name and number for dentist and doctor.
Name, number, and address for 3 emergency contacts.
Benefit History for Food Stamps (if applicable)
Evaluation or IEP (if applicable)
Head Start can not accept incomplete packets. If you do not have all of the documents listed above, please call Vera Manuel at 595-6391 and she will help you obtain the documents needed.
