How To Apply:: Preschool Registration March 17th- 9:00 AM to 12:00PM March 19th-23rd- 9:00 AM to 2:00PM Preschool is for children who will be three (3) or four (4) years old on or before September 1, 2012 . Documents needed to register for Preschool (Head Start or Pre-K) Â¨ Birth Certificate Â¨ Immunization Certificate (Form 3231) Â¨ Social Security Card Â¨ Proof of Income (example: 4 Pay Stubs 2011 Tax Return or proof of TANF) Â¨ Insurance, Medicaid, or Peach Care Card Â¨ Proof of Residency (Lease, mortgage or utility bill) ** Please do not bring Telephone Bills** (Please note that the three year old program is Head Start only and is based on the family meeting certain requirements) *A complete program is available for children with learning problems and disabilities*
